You are receiving this letter as a part of Humanistic Foundation Incorporated’s commitment to the privacy of your protected health information and to comply with certain privacy and security laws.

WHAT HAPPENED? 

On July 31, 2023, a third-party platform (Exym) notified us that they accidentally exported data containing 33 individuals' personal information; different data was supposed to be exported. The mistake was noticed within 10 minutes of receipt of the data, and Exym confirmed to us that the data was deleted less than 90 minutes after receipt.

WHAT INFORMATION WAS INVOLVED? 

The Export contained private client data from Humanistic Foundation Inc. The information that was released included names, ID’s, gender, status, ethnicity, age and diagnosis.

WHAT WE ARE DOING? 

We are in contact with LA County Department of Mental Health & Exym to make sure this doesn't happen again.

WHAT CAN YOU DO? 

If applicable, we also encourage you to place a fraud alert on your credit reports with the national credit bureaus and request a free credit report to ensure that accounts have not been established without your knowledge.
